Massage professionals please note: A complete application includes a signed and dated application on which complete, true, and accurate answers to all of the questions have been provided, as well as payment, passport photo, copy of government issued ID, original transcripts sent directly from school(s), exam proof sent directly from examining board, and live scan fingerprinting results have been received. Applications with issues, including but not limited to those received from applicants with criminal convictions, civil or administrative citations or where local action has been taken against the applicant, complaints have been made in relation to an applicant's professional conduct, or those with education issues can take a significantly longer period of time. Requirements for Certification

In order to obtain CAMTC Certification as a Certified Massage Therapist, an applicant shall submit an application in its entirety and provide satisfactory evidence that he or she meets all of the following requirements:

• The applicant is 18 years of age or older.
• The applicant has successfully completed the curricula in massage and related subjects totaling a minimum of 500 hours, or the credit unit equivalent, that incorporates appropriate school assessment of student knowledge and skills.
  
  
  • Of the 500 hours, a minimum of 100 hours of instruction shall address anatomy and physiology, contraindications, health and hygiene, and business and ethics.
  • All of the 500 hours shall be from a CAMTC approved school program.
    
    
    • Closed School Policy: For applications received after December 30, 2016, CAMTC will not accept any education from a California closed school unless that school 1) had CAMTC School Approval at the time of closure and remains in good standing, or 2) that school closed and remains in good standing with a CAMTC Approved School as its official Custodian of Records.
    • Out-of-State Education: Education and training received outside of California must be at least substantially equivalent to the requirements applied to California school programs. One important aspect of educational program equivalence is whether the educational institution attended has been approved by the national, regional, or state authority with responsibility for vocational program approvals. Such status can readily be ascertained by CAMTC for each of the 50 U.S. states, U.S. territories, and the Canadian provinces of Ontario, British Columbia, Nova Scotia, and Newfoundland and Labrador. Officially sealed transcripts from duly locally approved educational institutions located within those jurisdictions shall be sufficient school material for CAMTC to evaluate.
    • Out-of-Country Education: For education obtained outside of the 50 U.S. states, U.S. territories, and the Canadian provinces of Ontario, British Columbia, Nova Scotia, and Newfoundland and Labrador as outlined above, please read the Policies Regarding Massage Education Received Outside the United States.

• The applicant has passed one of the following CAMTC approved exams:

• Massage and Bodywork Licensing Exam (MBLEx)
• Board Certification Exam in Therapeutic Massage and Bodywork(BCETMB)
• National Certification Exam for Therapeutic Massage and Bodywork (NCETMB) – Taken on or before February 2015
• National Certification Exam for Therapeutic Massage (NCETM) - Taken on or before February 2015
• New York State Massage Therapy Examination

• The applicant has successfully passed a Live Scan fingerprint background investigation specific to CAMTC and has not violated any of the provisions of the California Massage Therapy Act or any policy, procedures, or regulations of CAMTC.
• All fees required by the council have been paid.

For applications received after December 30, 2016, CAMTC will not accept any education from a California closed school unless that school 1) had CAMTC School Approval at the time of closure and remains in good standing, or 2) that school closed and remains in good standing with a CAMTC Approved School as its official Custodian of Records.

CAMTC LIVE SCAN FINGERPRINTING INSTRUCTIONS

All applicants for CAMTC Certification in the profession of Massage Therapy are required to submit fingerprint information to the California Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI) via the Live Scan process.

Previously processed fingerprint cards or photocopies of fingerprint impressions are not acceptable. Applicants who have been fingerprinted via Live Scan by other agencies or for other purposes must be Live Scan fingerprinted again in order to supply the agency specific information required by the CAMTC.

Live Scan fingerprints must be taken in the state of California.

The DOJ and FBI fingerprint processing fees are established by the DOJ and FBI respectively, and are subject to change by those agencies without notice. A processing fee is also charged by the Live Scan Vendor. A typical total for fingerprint processing fees related to CAMTC Certification will range between $70 to $90, and may be broken down as follows:

• DOJ FINGERPRINT PROCESSING FEE $32.00

• FBI FINGERPRINT PROCESSING FEE $19.00

• LIVE SCAN VENDOR FEE $19 - $39

The local Live Scan Vendor will inform you of accepted methods of payment.

All applicants for CAMTC Certification must complete and submit the pre-populated Massage Therapist “Request for Live Scan Service form".

1. Make two (2) photocopies of the Massage Therapist form.

2. Choose a local Live Scan Vendor from the DOJ list.

Live Scan fingerprints can be obtained at most local Police and Sheriff stations, and other approved live scan locations. NOTE: Police and Sheriff stations may be more experienced in securing legible fingerprints from massage professionals with thin or worn ridges in the pattern area.

3. Take your completed, original Live Scan Request form and your two copies to the Live Scan Vendor of your choice.

4. Bring your Driver's License and/or other form of photo identification with you (passport, military ID, etc.).

5. Provide the Live Scan Fee to the Live Scan Operator (expect to pay between $70 to $90).

6. Once your fingerprints have been scanned, the Live Scan operator will complete the last section of your Live Scan Request form and will keep the original form.

7. One copy of the completed form is for your records. Submit the other completed copy of the Live Scan Request form to CAMTC. You can fax to 916-669-5337 or mail to CAMTC, 1 Capitol Mall - Suite 800, Sacramento, CA 95814

Your CAMTC Live Scan Request form has been pre-populated with the CAMTC identifying information required by the DOJ. The only section you need to complete is the “Applicant Information” section of the form.

Please take care to note the following as you complete the “Applicant Information” section of your Live Scan Request form:

• Your name on the Live Scan Request form must be identical to the name you provided on your application to the CAMTC.

• To facilitate prompt and accurate processing, please PRINT LEGIBLY as you fill in the “Applicant Information” section of the Live Scan Request Form.

The DOJ is able to process up to 95% of Live Scan Applicant fingerprint submissions in seventy-two (72) hours or less. Information from the FBI can take considerably longer.

to process. Fingerprints are rejected on occasion due to the applicant's fingerprints having characteristics that are difficult to capture in the live scan process. If your fingerprints are rejected by DOJ/FBI, CAMTC will notify you via mail with instructions on how to be re-printed.

Please note that the CAMTC expressly cannot guarantee that you will receive your CAMTC Certification on a specific date or within a specific time period, due to elements of the certification process that are beyond the control of the CAMTC. Such elements include, but are not limited to receipt of official school transcripts, receipt of supplementary information (if applicable), and receipt of fingerprint results from the DOJ and FBI application and certification process for massage professionals in California, and your feedback and support is greatly appreciated.

EXPIRED CERTIFICATE HOLDERS - YOU CAN STILL RECERTIFY!
No additional education or exam required.

As of July 1, 2016, CAMTC has instituted a new one-time policy for a very limited time only (until June 30, 2017), allowing past certificate holders (CMPs and CMTs) who expired on or after January 1 of 2013 to re-certify more than six months after the expiration of their certificates, as long as they meet the following requirements.

Who Does This Policy Apply to?

1. This policy only applies to those who were previously certified by CAMTC as a CMP or a CMT. It does not apply to those who were previously certified as a CCMP when their certificate expired.
2. This policy only applies to those whose certificates have been expired for more than six months and whose certificates expired on or after January 1 of 2013. It does not apply to former certificate holders whose certificates expired before January 1, 2013.
3. This policy only applies to certificate holders whose certificates were in good standing at the time of expiration. It does not apply to those who have had their CAMTC certificate revoked or nullified or to those whose certificates were suspended at the time of expiration. It does not apply to past certificate holders who have applied for recertification or certification and had their application for recertification or certification denied.

How Long Will This Second Chance Policy Be in Effect?

1. This policy will only be in effect for the limited time period of July 1, 2016 through June 30, 2017 at 11:59 p.m. (PST).

What Are the Requirements That Must Be Met In Order For This Policy to Apply?

1. Applicants must have been certified by CAMTC as a CMP or a CMT and had a certificate that was in good standing when it expired (not suspended, revoked, or nullified, or application denied).
2. CMP or CMT certificate must have been expired for more than six months and certificate must have expired on or after January 1 of 2013.
3. CAMTC must RECEIVE all of the following on or after July 1, 2016 and by or before 11:59 p.m. (PST) on June 30, 2017 (a postmark date will not suffice) in order for this policy to apply:
   • A completely filled out Limited Application for Recertification and all supporting documents;
   • All fees have been paid; and
   • A new Livescan must be performed and submitted.
   If all required items are not received by or before June 30, 2017, the requirements of this policy will not have been met and CAMTC will consider the application to be incomplete and the file will be purged, though the individual may still reapply as a new applicant. All fees, including the late fee, are non-refundable.
   
   Please note, CAMTC recommends that the application and all supporting documents be mailed well in advance of the June 30, 2017 cut off date, as June 30, 2017 is the last date that documents must be received in order for this policy to apply, regardless of the date documents were post-marked or mailed.
4. The following fees apply to this Limited Application for Recertification:
   • All regular application fees, including but not limited to the $150 fee for the application; and
   • An additional $125 late fee.
5. Applications received pursuant to this policy will be processed in accordance with CAMTC's standard operating procedures for applications for recertification. This policy only allows for the late receipt of applications for recertification and has no bearing on whether the application received will be approved or denied.
